Client intake

The health questionnaire and consents a client completes before using their account.

Intake is the questionnaire and signature a personal-training client completes the first time they sign in. Until it is submitted they cannot reach the rest of their account — the app sends them straight back to the form. That guarantees you never program for someone whose medical history you have not seen.

What the client fills in

The form opens with Welcome — let's get you set up and runs top to bottom in sections.

Personal — date of birth, sex assigned at birth, height in inches, weight in pounds, and occupation.

Medical history — a checklist of current or past conditions covering hypertension, type 1 and type 2 diabetes, heart disease, asthma or COPD, osteoporosis, arthritis, thyroid disorder, PCOS, anxiety or depression, autoimmune disorder, cancer history, diastasis recti, herniated disc, and None of the above, followed by a free-text box for anything not listed.

Medications — whether they take anything that may affect exercise, and if so, name and dosage.

Pregnancy / postpartum — currently pregnant, postpartum under twelve months, or not applicable.

Physician restrictions — whether a doctor has placed restrictions on exercise, a description if so, and the physician or provider name and phone.

Surgeries / injuries — repeatable rows added with Add injury / surgery. Each row takes a description, a year, a side (left, right, both, or not applicable), and an Active? checkbox for whether it is still an issue.

Chronic pain — whether they experience it, then a body-region multi-select (neck, shoulders, upper back, lower back, elbow, wrist, hips, knees, ankle or foot) and a free-text box for location, severity, and duration.

Additional questions — anything you have added yourself (see below).

Waivers & signature — every published waiver that applies to personal-training clients, then the consent checkboxes and a field to type their full legal name.

Two separate consents

Both are required, and they are deliberately not combined:

  1. The waiver agreement — "I have read and agree to the waiver above", confirming the information given is accurate. This is between the client and you.
  2. Health-data consent — a distinct checkbox giving Praevio permission to process the health information above. It links to the Consumer Health Data Privacy Policy and is recorded with a version, so it is clear exactly what was agreed to and when.

Optional waiver documents get their own I agree to this optional release checkbox; leaving one unchecked declines it and does not block submission. Every medical field is optional — a client can leave anything blank. Sign & Submit completes intake.

Your custom questions

Open Settings and find Additional intake questions. Add question takes the question text, a type (Short text, Long text, Single choice (radio), or Multiple choice (checkboxes)), the options where the type needs them, and a Required? toggle. Use Move up and Move down to reorder. Your questions appear after the standard health section on every client's intake.

Reading and resetting a response

The client's Intake tab shows the submitted response grouped the same way as the form, with a Signed waiver block naming the signer and the date, View signed waiver text to read the exact wording they agreed to, and Print record. If intake has not been completed yet the tab reads Intake pending — the client will complete this on first login.

Request re-take clears the response so the client is prompted again.

Requesting a re-take permanently deletes the client's current intake response. Print or note anything you need first.
